Time and material work priced after the fact is priced by whoever argues hardest, and that's never the subcontractor. It's also the only way equipment gets paid for on T and M at all: a machine with no published rate becomes a machine you brought for free. Replace the example rates with yours. They're real numbers from a client engagement and they need vetting for your region, your wage determination and what you paid for the iron.
Put fuel in as gallons per hour per machine rather than one blended figure. An excavator at six and a loader at sixteen are different costs.
Get it attached to the subcontract. A rate sheet referenced in the contract is binding; the same sheet sent after the first change is a proposal.
